MEKANISME KERJA ANTIBIOTIK<\/p>\n

1. Mekanisme kerja antibiotic melalui penghambatan sintesis dinding sel<\/p>\n

Dinding sel merupakan lapisan luar sel bakteri yang berfungsi mempertahankan bentuk sel dan pelindung sel bakteri yang memiliki tekanan osmotic internal yang lebih tinggi daripada lingkungannya. Tekanan osmotic internal bakteri gram positif lebih besar 3 hingga 5 kali daripada tekanan osmotick.<\/p>\n

Internal bakteri gram negatif. Penghambatan sintesis dinding sel menyebabkan sel lisis. Dinding sel bakteri mengandung peptidoglikan yang secara kimia berisi polisakarida dan campuran rantai polipeptida yang tinggi. Polisakarida dari peptidoglikan berisi gula amino N-acetylglucosamine dan asam acetylmuramic. Sifat keras pada dinding sel disebabkan oleh hubungan saling silang rantai peptide (seperti melalui ikatan pentaglycine) yang merupakan hasil reaksi transpeptidasi yang dilakukan oleh beberapa enzim. Semua \u03b2-lactam menghambat sintesis dinding sel bakteri dengan berikatan pada reseptor sel (beberapa merupakan enzim transpeptidase).<\/p>\n

Reseptor yang berbeda memiliki afinitas yang berbeda terhadap antibiotic. Protein reseptor ini berada dibawah control kromosom, sehingga mutasi dapat mengubah jumlah atau afinitas reseptor terhadap antibiotic \u03b2-lactam. Setelah \u03b2-lactam melekat pada satu atau beberapa reseptor, reaksi transpeptidasi dihambat dan sintesis peptidoglikan dihentikan. Kemudian terjadi perpindahan atau inaktivasi inhibitor enzim otolitik pada dinding sel. Aktivitas enzim litik akan enyebabkan lisis jika lingkungan isotonic. Penghambatan enzim tranpeptidase oleh penisilin dan sefalosporin menyebabkan hilangnya D-alanine dari rantai pentapeptida dalam reaksi transpeptidasi.<\/p>\n

2. Mekanisme kerja antibiotic melalui hambatan fungsi membran sel<\/p>\n

Membrane sel bakteri berfungsi sebagai barrier permeabilitas selektif, berperan dalam transpor aktif dan mengontrol komposisi internal sel. Ketika fungsi integritas membrane sel dirusak maka makromolekul dan ion akan keluar dari sel, kemudian sel rusak dan mati. Antobiotik yang menghambat fungsi membrane sel akan berikatan dengan sterol yang terdapat pada membrane sel bakteri<\/p>\n

3. Mekanisme kerja antibiotic melalui penghambatan sintesis protein<\/p>\n

Aminoglikosida merupakan salah satu antibiotic yang menghambat sintesis protein. Penghambatannya melalui penambahan aminoglikan pada reseptor protein spesifik pada subunit 30S ribosom bakteri. Kemudian aminoglikosida akan memblokir aktivitas inisiasi kompek normal pembentukan peptide (mRNA+Formyl methionine+tRNA). Selanjutnya akan terjadi salah pembacaan daerah pengenalan ribosom secara konsekuen asam amino oksalat dimasiukan kedalam peptide sehingga menghasilkan protein fungsional. Selanjutnya penambahan amino glikosida berakibat, dalam pemecahan polisom menjadi monosom yang tidak dapat mensintesis protein.<\/p>\n

4. Mekanisme kerja antibiotic melalui penghambatan asam nukleat<\/p>\n

antibiotik seperti rifampin akan menghambat pertumbuhan bakteri dengan ikatan yang sangat kuat dengan enzim DNA Dependent RNA polymyrase bakteri, sedangkan antibiotik trimetoprim akan menghambat sintesia asam nuklet melalui penghambatan enzin reduktase dihidrofolat, enzim ini mereduksi dihidrofolik terhadap asam tetrahidrofolat, yang berperan dalam sintesis purin dan DNA.<\/p>\n
